PREFACE

Manual «Case History of therapeutic patient» is proposed for the correct assimilation of the subject «Introduction to Internal Diseases», effective work at the patient's bedside, and writing the first student's case history.

Despite the greater importance of patient surveys technologically, classical methods of diagnostics - detailed questioning and careful consistent objective examination - remain important («conditio sine qua non»). Possession of these techniques and their effective application, clearly defines the professional culture of doctors, and also creates a special confidential atmosphere of patient - physician interaction - the compliance, on which success of treatment largely depends.Unfortunately, most of the doctor's mistakes stems from the neglect of these classic methods of investigation: patients poll superficially, cursorily and often inconsistently are inspected. A famous Russian physician M.V. Cher-norutsky said: «One mistake from ignorance represents to nine from inattention».

The proposed scheme of the patient examination is based on the thousand-year experience of medical help to avoid mistakes, especially at the beginning of the difficult path of healing. It should be noted that in Europe the detailed record of cases was introduced in the XVII century by Dutch physician Herman Burgav and in Russia in the XIX century by M.Y. Mudrov, «father» of Russian therapeutic school. M.Y. Mudrov wrote: «The history of the disease should have the dignity of the exact story of what happened... the history of the disease should be written cost-conscious at bedside... This treasure is dearer to me than my whole library».
